To request a copy of your records, Please complete a Medical Records Release Form available at the front desk.

Please note:

• ·        A valid photo ID is required
• ·        Requests must be signed by the patient or authorized representative
• ·        Processing
• ·         time may take up to 7 business days
• ·        Fees may apply in accordance with Texas state guidelines

Records may be:

• ·        Picked up in person
• ·        Faxed to another provider
• ·        Mailed to the address provided
